29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    a) For the construction site at 2810 N 35th St., Tampa, FL 33605, as occurring on or about 05/02/2015, employees exposed to ladder fall hazards were not able to recognize ladder hazards and had not been trained/instructed on procedures for safe usage of ladders. Employees used extension ladders on site that did not extend at least 3 ft. above the upper surface edge that was being accessed.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  a) For the construction site at 2810 N 35th St., Tampa, FL 33605, as occurring on or about 05/02/2015, employees exposed to ladder fall hazards were not able to recognize ladder hazards and had not received training on procedures for safe usage of ladders. Employees used extension ladders on site that did not extend at least 3 ft. above the upper surface edge that was being accessed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) For the construction site at 2810 N 35th St., Tampa, FL 33605, as occurring on or about 05/02/2015, employees were exposed to fall hazards of 14 ft. from unprotected floor edges on either side of a hallway being accessed. On 05/02/2015, an employee fell from the unprotected edge while in the process of unloading drywall sustaining injuries and was hospitalized.

The extension ladder rails did not extend at least 3 ft. above the upper edge exposing employees to fall hazards of 14 ft.
