29 CFR 1926.501(a)(2): The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. On or about May 12, 2015, at 1002 Washington St. Portsmouth, a single story commercial building, a roofer was exposed to a fall through hazard of 15 1/2 feet due to the lack of adequate testing of the roof deck working surface to ensure that it would support employees while working on the roof.

29 CFR 1926.502(a)(2): Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection. On or about May 12, 2015, at 1002 Washington St. Portsmouth, a single story building, roofers were exposed to a 15 1/2 foot fall hazard due to the lack of a fall protection system installed before roofers began removing and installing 3 ft. x 20 ft. metal decking over the roof joists.

On or about May 1, 2015, at 1002 Washington St., Portsmouth, a one story commercial building, roofers were exposed to a 15 ft. 1/4 inch fall hazard while removing the existing gypsum board decking, creating a hole, and then installing the 3' x 21' metal decking in its place without utilizing any fall protection.

On or about May 12, 2015, at 1002 Washington St., Portsmouth, a single story commercial building, roofers were exposed to a 15 ft. 1/4 inch fall hazard by not utilizing any fall protection while installing 3 ft. x 21 ft. metal roof decking over the roof joists/purlins.
