On or about May 13, 2015 machine operators were exposed to point of operation hazards in the production area where the employer did not guard the moving parts of the AIM wire bending machine. c. On or about May 13, 2015 machine operators were exposed to point of operation hazards in the production area where the employer did not guard the moving parts of the BMS-02 wire bending machine. d. On or about May 13, 2015 machine operators were exposed to point of operation hazards in the production area where the employer did not guard the moving parts of the B3-01 wire bending machine. e. On or about May 13, 2015 machine operators and quality assurance employees were exposed to electrical hazards in the production area where the employer used flexible cords to provide power to the quality control table light in the output control area adjacent to the B3-01 area. c. On or about May 13, 2015 machine operators and quality assurance employees were exposed to electrical hazards in the production area where the employer used flexible cords to provide power to the B3-01 quality control table light. d. On or about May 13, 2015 machine operators and quality assurance employees were exposed to electrical hazards in the production area where the employer used flexible cords to provide power to the B3-02 quality control table light. e.
