29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. a). Front Radius - On or about May 13, 2015, an employee's upper body, arms, and hands were not adequately protected from the sparks and heat generated during the tack welding operation of a piece of perimeter angle steel, thus exposing the employee to burn injuries. b). Front Radius - On or about May 13, 2015, an employee was not wearing adequate eye protection when using a DeWalt VSR All-Purpose Scrugun (model: DW257) to install metal decking, thus exposing the employee to eye injuries.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a). Front Radius - On or about May 13, 2015, employees were installing metal decking while working from the front radius without any fall protection system in place, thus exposing the employees to a fall hazard of approximately 20 feet onto concrete ground. Fabrication Solutions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) , which was contained in OSHA inspection number 1001478, citation number 1, item number 1 and was affirmed as a final order on December 8, 2014, with respect to a workplace located at 2049 Pat Thomas Parkway, National Guard Armory, Quincy, FL 32352. Fabrication Solutions, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 313877961, citation number 1, item number 1 and was affirmed as a final order on October 1, 2010, with respect to a workplace located at Success Drive - Chipola College, Marianna, FL 32444.
