29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use        a) Paint Booth, employees were required to use half face tight fitting respirators while spray painting paints and clear coatings exposing employees to inhalation hazards without a written respiratory protection program.        b) Paint Booth, employees were required to use a half face tight fitting respirator while spray painting without a medical exam exposing employees to physiological hazards.        c) Paint Booth, employees were required to use a tight fitting half face piece respirator while spray painting without a fit test prior to initial use.

29 CFR 1910.134(d)(3)(i): The employer did not provide a respirator that was adequate to protect the health of the employee under routine and reasonably foreseeable emergency situations:    Paint Booth, employees were exposed to hexamethylene diisocyanate homopolymer wearing 3M air purifying half facepiece respirators as required by the employer. The appropriate respirator for employees routinely exposed to hexamethylene diisocyanate homopolymer is a supplied-air respirator.
