Exterior of the Building - On or about May 13, 2015, personal fall protection equipment was not worn by employees installing exterior sheathing while working from a Genie Articulating Boom Lift (model: Z-45/25), thus exposing employees to a fall hazard of up to 18.5 feet onto earthen ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a). Interior of the Building - On or about May 13, 2015, employees gained access to the roof by using the overhead metal decking of a storage room that did not have a fall protection system in place, thus exposing employees to a fall hazard of approximately 10.75 feet onto metal wire mesh and earthen ground.
