29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  (a) Facility- 400 Technology Drive, Coal Center, PA: On or about May 12, 2015, employees were not required to de-energize and isolate the ZES breaker which ultimately exposed the workers to the operation or the rubber roll and antilock roll when the Wade Die Cutter was opened.  OR IN THE ALTERNATIVE:  29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Facility- 400 Technology Drive, Coal Center, PA: On or about May 12, 2015, employees were exposed to ingoing nip points from the antilock roll and the rubber roll due to lack of guarding.
