29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Shipping/Receiving Area - On or about May 14, 2015, Machined Metals Manufacturing, Inc. did not ensure that each powered industrial truck operator was competent to operate the Komatsu forklift truck safely as demonstrated by the successful completion of training and evaluation. In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.242(b): Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment. a) Shop Area - On May 14, 2015, Machined Metals Manufacturing, Inc. did not ensure compressed air used for cleaning purposes (cleaning machined parts) was reduced to less than 30 p.s.i. and then only with effective chip guarding at the HAAS turning center (M21). In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification.
