29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).      On or about May 14, 2015 at the Buddy Holly Center East building, employees were exposed to fall hazards when operating a boom lift without being tied off to the basket while working on roofing activities .

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.         On or about May 15, 2015, on the Main entrance of the Buddy Holly Center, employees who where installing roofing material where not tied off, exposing employees to the hazard of falling approximately 12.6 feet to the ground bellow.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.    On or about May 15, 2015, on the East side building of the Buddy Holly Center, employees who where installing roofing material on a steep roof where not tied off, exposing employees to the hazard of falling from a height of more than 6 feet to the ground bellow.
