29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a)  On the Garage Shop Roof - On or about May 7, 2015, employees, installing screws in the metal roofing material on a 1.5:12 pitch roof, were not protected from a fall hazard of 17.4 feet by the use of a guardrail system, safety net system, personal fall arrest system, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      b)  On the Storage Area Roof - On or about May 7, 2015, employees, installing metal roofing on the storage area's 1.5:12 pitch roof, were not protected from a fall hazard of 11.8 feet by the use of a guardrail system, safety net system, personal fall arrest system, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  On the Garage Shop Roof - On or about May 7, 2015, employees, installing screws in the metal roofing material on a 1.5:12 pitch roof, were not adequately trained to recognize the nature of the 17.4-foot fall hazard to which they were exposed without the use of a fall protection system.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  At the East side of the Northeastern Corner - On or about May 7, 2015, the ladder used to access and egress the northeastern corner of the 1.5:12 pitch roof did not extend 3 feet above the edge, exposing employees to a fall hazard of 11.8 feet.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
