29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.   a) LSS Site, Sand Blasting Booth, Catwalk: On or about May 13, 2015, an employee was sandblasting a vessel/tank while standing on a catwalk (above-ground work platform) with unguarded open sides, exposing the employee to a 5-foot fall hazard.     b) LSS Site, Paint Booth, Catwalk: On or about May 13, 2015, an employee was painting a vessel/tank while standing on a catwalk (above-ground work platform) with unguarded open sides, exposing the employee to a 5-foot fall hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips.  a) The Main Building, Machine Shop work area: On or about May 13, 2015, an employee used a DoAll Vertical Band saw which had a blade that was not completely guarded, exposing the operator/employee to a point-of-operation hazard.     b) The LSS Site Building, Vacuum-Jacket Work area: On or about May 13, 2015, an employee used a Marvel-Mark III Vertical Band saw which had a blade that was not completely guarded, exposing the operator/employee to a point-of-operation hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) Main Building, Dip Tank Work Area: On or about May 13, 2015, an employee used an electric hoist which had a cord that was not connected to accommodate strain relief, exposing employees to an electrical hazard.
