On or about May 14, 2015, located at 704 Washington Blvd in Belpre, Ohio, employees were using a pneumatic nailer without eye protection, thereby exposing employees to a stuck-by hazard.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. On or about May 14, 2015, located at 704 Washington Blvd in Belpre, Ohio, the employer failed to ensure employees were protected from fall hazards while preforming roofing work on a single story, six on twelve pitched firehouse. Employees were exposed to a maximum fall hazard of approximately 22 feet.
