29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard: Worksite located at Singleton Trust No. 2. Grady County, Oklahoma. Rig #7. On or about May 14, 2015 and at times prior thereto, the drilling operator platform was not provided with a mid-rail chain, exposing the driller to a fall from height of more than 4 feet to the ground level below.
