29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about May 6, 2015, at 1200 Pinnacle Parkway, the employer failed to provide fall protection for employees who were engaged in roofing activities on top of an awning at a height of 13 feet or greater.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: On or about May 6, 2015, at 1200 Pinnacle Parkway, the employer allowed employees to use a 10-foot A-frame ladder for access/egress of an awning that measured 13 feet in height from ground below. The employees were using the top step of the 10-foot ladder to access the awning.
