Memorial Bridge Rotary, West Springfield, MA: On or about May 15, 2015: an employee was observed painting structural steel members more than 12 feet above the next lower level (ground) without being protected from falling by the use of guardrails, safety nets systems or a personal fall arrest system. The Agnoli Sign Company was previously cited for a violation of this occupational safety and health standard 29CFR1926.501(b)(1), which was contained in OSHA inspection number 313207995, citation number 1, item number 1 and was affirmed as a final order on January 11, 2011, with respect to a workplace located at 1500 Main Street Springfield, MA.
