29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about May 13, 2015, at 409 Madison Park Drive, Grayson, Georgia:  Eight employees performing roofing work installing asphalt shingles were exposed to struck-by hazards from nails, wood chips, and shingle debris while operating nail guns without using eye protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 13, 2015, at 409 Madison Park Drive, Grayson, Georgia:  Nine employees performing roofing work on an approximately 12/12 slope roof were exposed to fall hazards of approximately 23.7-feet and greater when the employer did not instruct them how to recognize and minimize fall hazards.
