29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Location - 200 Springs Road, Building 5, Bedford, MA: On or about 5/13/2015, steam supply and return valves were not locked or tagged out during the repair of the buildings heating system. Veterans Health Administration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 314652629, citation number 1, item number 8 and was affirmed as a final order on September 10, 2010, with respect to a workplace located at 17273 St. Rt. 104, Chillicothe , OH 45601 .
