OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    (a) Halliburton Energy Services, Inc. at 3199 D Road, Grand Junction, CO:  On or about April 30th, 2015, an employee was exposed to a rotating part while removing and replacing a suction pump on blender unit #FB4K.  While troubleshooting a reported noise and vibration issue, the 8 inch screw cap on the suction line was removed to access an energized 12 inch master suction valve.  An employee reached into the 12 inch master suction valve, while the blender unit was deenergizing, attempting to find and remove a suspected foreign object when the valve closed resulting in an amputation of 4 fingers.  The condition exposed an employee to an amputation hazard.
