29 CFR 1915.71(b)(7): Scaffolds were erected, moved, dismantled or altered without the supervision of competent persons. a) 2355 Jessie Dupont Highway, Burgess, VA 22432, Vessels Ciel and Wasting Aweigh: On or about May 18, 2015, and times prior thereto, scaffolds erected alongside the hulls of vessels to prevent a fall of greater than 5 feet from the boat deck to a lower level, while employees performed fiberglass work and replaced portlights, and to provide a work platform to varnish trim at heights with an approximately 75" platform to ground fall distance, were not erected or altered under the direction of a competent person qualified in scaffold erection, moving, dismantling and alteration. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1915.71(j)(1): Scaffolding, staging, runways, or working platforms which are supported or suspended more than 5 feet above a solid surface, or at any distance above water, were not provided with a railing, which had a top rail whose upper surface was from 42 to 45 inches above the upper surface of the staging, platform, or runway and a midrail located halfway between the upper rail and the staging, platform, or runway. a) 2355 Jessie Dupont Highway, Burgess, VA 22432, Vessel Wasting Aweigh: On or about May 18, 2015, and times prior thereto, each employee who performed work from tubular frame scaffold with an approximately 75" platform to ground fall distance, was not protected from fall hazards by backrails that were up to 18 inches horizontal from the working platform. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
