29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: Construction phase 4, stair area A, west: On or about May 19, 2015, and at times prior thereto, employees were exposed to a fall hazard greater than six feet to the ground, while installing a guardrail system around an open hole between the second floor and the first floor, without being trained on fall hazard recognition.
