29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) On or about May 18, 2015 and at times prior, exit doors were installed with a locking mechanism and would be locked sometimes to prevent outsiders from getting in. Employees were exposed to possible entrapment in the event of fire emergency.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: a) On or about May 18, 2015 and at times prior, an 8 inch wheel grinding machine was not provided with a tongue guard. An employee used the grinding machine and was exposed to possible flying particles from grinding drill bits.
