29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 19, 2015, at 29103 Bambi Place, Boerne, TX, an employee was observed working below another employee using an air gun to install metal screening. Employee working below did not have head protection. Employee exposed to the hazard of being struck by falling objects.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights:  On or about May 19, 2015, at 29103 Bambi Place, Boerne, TX, an employee was observed working from a scaffolding that was not fully planked. The employee was exposed to a fall hazard of approximately seven feet and five inches.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  On or about May 19, 2015, at 29103 Bambi Place, Boerne, TX, the employer failed to ensure the employees had safe access when ascending and descending the scaffolding by providing an approved ladder. The employees were exposed to a fall hazard of approximately seven feet and five inches.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about May 19, 2015, at 29103 Bambi Place, Boerne, TX, an employee was exposed to a six foot fall hazard. The employee was stepping on and off a free-standing, portable ladder to gain access to the scaffolding. The Louisville step ladder was used in a manner that is prohibited by the manufacturer.
