29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a)  At the Worksite:  101 Essex Street, Harrison, NJ.  Roofing workers were exposed to falls of approximately 25 feet to the floor of the building below as the plastic skylight was not protected by screens or other material which would prevent workers from breaking through the covering if a fall were to occur.                                                               Violation observed on or about 05/07/2015.

29 CFR 1926.501(b)(10):    Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)  At the Worksite:  101 Essex Street, Harrison, NJ.  Roofing workers were exposed to falls of approximately 25 feet to the ground below as low-slope roof fall protection methods such as but not limited to safety monitoring and personal fall arrest systems were not in use when workers were outside the warning line between the line and the roof's edge.                                                              Violation observed on or about 05/07/2015.

29 CFR 1926.502(f)(2)(ii):   The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface:  a)  At the Worksite:  101 Essex Street, Harrison, NJ.  Roofing workers were exposed to falls of approximately 25 feet to the ground below as the fall protection warning line was not rigged properly in that in the Southeast and East areas of the roof the line was either laying on or close to the roof's surface.                                                          Violation observed on or about 05/07/2015.

29 CFR 1926.503(a)(1):   The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a)  At the Worksite:  101 Essex Street, Harrison, NJ and at the Corporate Office at 56 Summit Avenue, #1, Elmwood Park, NJ.  Roofing workers who were exposed to falls did not have instruction from this company on fall hazard recognition and fall prevention relating to 25 foot falls from the roof and through skylights.                                                               Violation observed on or about 05/07/2015.
