29 CFR 1926.502(f)(1)(iv): When the path to the point of access was not in use, a rope, wire, chain, or other barricade, equivalent in strength and height to the warning line, was not placed across the path at the point where the path intersected the warning line erected around the work area, or the path shall be offset such that a person cannot walk directly into the work area. On or about 05/18/2015 at the Loos & Company Re-Roofing Project, employees were accessing the work area from a ladder and were also accessing the dumpster area and no barricade or rope or wire was placed across the path.

29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area: On or about 05/18/2015 at the Loos & Company Re-Roofing Project, employees were working on the roof deck using a warning line system with approximately 30 feet by 30 feet of the warning line system that was missing from the North/East corner.
