29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) Building 5, Mallard Court, Flemington, NJ 08822. Two employees were exposed to fall hazards when working on and from the roof of Building 5 with no fall protection systems in place. b) Building 7, Mallard Court, Flemington, NJ 08822. Two employees were exposed to fall hazards when working on and from the roof of Building 7 with no fall protection systems in place. Conditions occurred on or about 05/18/15.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: a) Building 7, Mallard Court, Flemington, NJ 08822. Employee were exposed to fall hazards when using a portable aluminum extension ladder to gain access to the roof when the ladder was not extended three feet above the roof landing. Condition occurred on or about 05/18/15.
