29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.  On or about October 3, 2015, the employer did not certify abatement which was to be completed by 09/22/2015 for Citation 2 Item 1 issued on 09/10/25:  29 CFR 1926.503(b)(1): The employer did not prepare a written (training) certification record that contained the name or other identity of the employee trained, the date(s) of the training, the signature of the person who conducted the training or the signature of the employer and the date the employer determined the prior training was adequate rather than the date of actual training when the employer relies on training conducted by another employer or completed prior to the effective date of this section.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about May 15, 2015 an employee  was exposed to an approximate 9' fall hazard while preparing to hang a sign at the Gables Apartments without using fall protection.

29 CFR 1926.503(b)(1): The employer did not prepare a written (training) certification record that contained the name or other identity of the employee trained, the date(s) of the training, the signature of the person who conducted the training or the signature of the employer and the date the employer determined the prior training was adequate rather than the date of actual training when the employer relies on training conducted by another employer or completed prior to the effective date of this section.  On or about May 15, 2015,  the employer could not provide certification that the exposed employee had received fall hazard awareness training.        Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.
