29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    (a)  350 N 18th Street, Lebanon Pennsylvania - An employee used a table saw to rip a piece of plywood.  The employer did not ensure that the blade was guarded by an automatically adjusting hood, on or about May 19, 2015.      No abatement certification or documentation required, corrected during the inspection.

The employer did not ensure that the spreader was in place to prevent material from being thrown back on the employee, on or about May 19, 2015.     No abatement certification or documentation required, corrected during the inspection.

The employer did not ensure that non kick back fingers or dogs were provided on the table saw when ripping would occur, on or about May 19, 2015. 29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA:    (a)  350 North 18th Street, Lebanon Pennsylvania - An employee suffered a hospitalization on May 13, 2015 as a result of a cut to the finger from a Ridgid table saw.  The employer failed to report this work-related hospitalization to OSHA within 24 hours.  (The employer reported this hospitalization to OSHA on May 15, 2015.)    No abatement certification or documentation required.
