29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about May 21, 2015 the employer failed to provide proper personal protective equipment to ensure employees were protected while being required to conduct welding operations in a wet and/or damp location. Employees were exposed to electric shock and/or death by electrocution.

29 CFR 1910.254(d)(6): Printed rules and instructions covering operations of equipment supplied by the manufacturers were not strictly followed: (a) On or about Thursday, May 18, 2015, the employer did not follow manufacturer's instructions regarding welding in wet or damp locations. This included, but was not limited to: Lincoln LN-25 suitcase welders and Lincoln Arc CV/CC DC-655 welders. Employees were exposed to electric burns, shock and/or death by electrocution.
