29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): (a) #233 Reese Run Rd. Lumberport, WV: Employees working in the excavation consisting of type B soil were exposed to a cave-in. The excavation was approximately 54 feet long 15 feet wide at the top 12 feet wide at the bottom and was approximately 11 feet deep. The excavation was not sloped to a 1 to 1 slope, benched, or shored as observed on or about 5/15/2015. The Schwob Energy Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(b)(1)(i), which was contained in OSHA inspection number 941949, citation number 1, item number 3 and was affirmed as a final order on November 26, 2013, with respect to a workplace located at 1660 Dixon Ridge Road, Marietta, OH, 45750. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
