29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     The employer does not ensure that employees use eye protection when machines or operations present potential eye or face injury from physical agents.  This violation was observed on or about May 15, 2015, on the east side of the structure where an employee was exposed to a struck-by hazard when using a Hitachi angle grinder to cut stone without using eye protection.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees use eye protection when machines or operations present potential eye or face injury from physical agents.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:     The employer does not ensure that when scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, or similar means shall be used.  This violation was observed on or about May 15, 2015, on the east side of the structure where an employee was exposed to a fall hazard when climbing the frame of a frame scaffold to gain access to the awning without using a portable ladder, hook-on ladder, attachable ladder, or other similar means.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure when scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, or similar means shall be used.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     The employer does not protect employees from falling when working six feet or more above a lower level.  This violation was observed on or about May 15, 2015, on the east side of the structure where an employee was exposed to a 10 foot 3 inch fall hazard when performing stone work without being protected from falling to the lower level.         Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls six feet or more above a lower level.
