Trucking and Excavating, Inc., 350 East 6th Street, Erie, PA  16507, on or about May 14, 2015 and times prior thereto:  Employees were using chemicals including, but not limited to, Shell RotellaT Triple Protection Heavy Duty Engine Oil, Stihl Woodcutter Bar and Saw Chain Lubricant, SuperS Super Trac 303 Tractor Hydraulic Fluid, gasoline and diesel fuel.  Copies of the material safety data sheets were not maintained for each of these hazardous chemicals.
