29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: (Construction Reference: 1926.59) A.) Commercial Industrial Insulation Services, Inc. dba C.I.I.S., Inc., 350 East 6th Street, Erie, PA 16507, on or about May 14, 2015 and times prior thereto: Employees did not receive training on the new labeling elements (pictograms) and safety data sheet format by the December 1, 2013 deadline noted in the 29 CFR 1910.1200 (j)(1) of the Hazard Communication - 2012 Standard. Employees were using chemicals including, but not limited to, Childers CP-10 VI-CRYL Weather Barrier Mastic for Sealing Insulation Surfaces.
