OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to crushing hazards: (a) On or about 05/12/2015 - in Bay A8, an unqualified employee who was operating a floor-operated 5-ton monorail underhung crane was caught-between the crawler base of a mobile mining fan and the concrete floor when the crawler base was loosened from the fan and fell to the floor.
