29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: At the General Mail Facility building center area, panel box RF-4, on or about May 14, 2015, and at times prior thereto, employees were exposed to electrical shock hazards of approximately 600 volts from an electrical distribution box that had that had an unprotected end of the conduit with a damaged fastener running onto the side of the box which is used to power equipment such as but not limited to an automatic facer canceler system.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) General Mail Facility building in the engine room within the tool room, Panel AJA, on or about May 14, 2015, and at times prior thereto, employees were exposed to electrical shock hazards of approximately 120~208 volts from unused openings along the bottom and sides of an electrical distribution panel box used to power mail sorting equipment and lighting. b) General Mail Facility building east side interior wall, Panel labeled AJ, on or about May 14, 2015, employees were exposed to electrical shock hazards of approximately 120~208 volts from unused openings along the bottom and sides of an electrical distribution panel box that powers mail sorting equipment and lighting. c) General Mail Facility building southwest area, Panel AAAB, on or about May 14, 2015, employees were exposed to electrical shock hazards of approximately 120~208 volts from unused openings along the bottom and sides of an electrical distribution panel box that powers mail sorting equipment and lighting. d) At the General Mail Facility building center area, Panel Labeled RF-4, on or about May 14, 2015, employees were exposed to electrical shock hazards of approximately 600 volts from unused openings along the bottom and sides of an electrical distribution panel box that powers equipment such as the automatic facer canceler system.
