29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent. On or about May 20, 2015, the employer did not correctly enter the following work-related injuries or illnesses on the OSHA Form 300 or equivalent: a) Case No. 15-4, an employees suffered head and shoulder pain from a slip/fall that resulted in a Job Transfer/Restriction. No days were entered into column (L) for the job transfer/restricted work day count. b) Case No. 15-7, an employee suffered low back strain from a lift that resulted in Days Away from Work recordable incident. No days were entered into column (K) and (L) for the days away from work and job transfer/restricted work day count. c) Case No. 15-8, an employee suffered a strain to the right arm/wrist from a repetitive motion injury that resulted in a job Transfer/Restriction recordable incident. No days were entered into column (L) for the job transfer/restricted work day count.
