29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about May 19, 2015, and at times prior thereto, at the establishment the employer did not establish a Lockout/Tag-out program consisting of an energy control procedure, employee training and periodic inspections in maintenance testing operations.  Employees were exposed to unexpected release of energy hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about May 19, 2015, and at times prior thereto, employees were allowed to operate machines that were found not having guards,  including but not limited to the following instances:     1) At the press brake area, press brakes being operated via a foot pedal without machine guard provided to protect operator and other employees from exposure to the point-of-operation while in use.     2) At the shear area, a shear operated with a blade guard provided however, the guards opening was sized  to allow fingers to enter to the point-of-operation while in use.     3) In the wood department, a radial arm saw motors guard was broken and would allow fingers to enter the rotating motor while in use.   Employees were exposed to  amputation hazards.

29 CFR 1910.213(b)(3): Provisions were not made to prevent woodworking machine(s) from automatically restarting upon restoration of power after power failure:    On or about May 19, 2015, and at times prior thereto, in the wood department employees were allowed to operate a radial arm saws that did not have an "anti-restart" device installed to prevent automatic restarting of the saw. Employees were exposed to amputation hazards.

29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals:    On or about May 19, 2015, and at times prior thereto, in the power press areas employees were allowed to operate mechanical power presses via a foot pedals but the foot pedals did not have a guard provided to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedal.  Employees were exposed to an amputation caused by unintended operation of the press.
