29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): a) Jobsite - On or about, and at times prior to May 10, 2015, employees accessed the 480-volt 3-phase panel box was located at the pivot of a Fulghum 155 Log Crane. The panel box was not de-energized to a zero state; in that the main service feed was not locked out at the main MCC room, exposing employees to electric arc and shock hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: a) Jobsite - On or about and at times prior to May 10, 2015, the employer did not ensure an employee who was trained and authorized to open panel boxes and open motor control boxes was provided with adequate PPE to protect from ARC flash and ARC blast protection, were designed and rated to protect employees from arc-flash hazards.

29 CFR 1910.335(a)(1)(ii): Protective equipment was not maintained in a safe, reliable condition and/or was not periodically inspected or tested: a) Jobsite - On or about and at times prior to May 10, 2015, the employer did not ensure employees who used Dielectric Gloves, Model: NOVAX ANSI/ASTM, Class O, D 120 Type 1, S/N 1127691, were designed and rated to protect employees from arc-flash hazards in that dielectric gloves were not inspected as required. The last test/inspection date displayed 2011, which exposed employees to electrical and arc-flash hazards.
