29 CFR 1926.403(b)(1)(iii): The employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees. At a residential construction site, the employer was using an extension cord that was damaged. The outer jacket was cut on various locations. This condition exposed employees to hazards associated with electrical shock.
