The conduit box was not mounted to the freezer and was missing knockouts. The conduit box was not designed for the application. Employees were exposed to electrical shock hazards.

An employee was shocked causing a breaker to trip. All the breakers in the electrical box were not legibly marked to indicate their purpose. Employees were exposed to electrical shock hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Kitchen Area - On or about May 20th, 2015, the cord of a stainless steel chest freezer was not inspected for external defects. The outer jacket of the cord was damaged exposing the live conductor. An employee was shocked and injured when he touched the energized stainless freezer.
