29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: a) Jobsite - Employees engaged in work around the entrance to the first floor stairwell were exposed to debris, including but not limited to loose lumber and lumber with protruding nails. Observed on or about 18 MAR 2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Jobsite - Employees took breaks between the warning line system and the edge of the roof. As a result, the employees were exposed to fall hazards of approximately forty (40) feet. Observed on or about 18 MAR 2015.
