29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions: On or about May 12, 2015 an employee mixing sand and epoxy into a mixer at 130 N. Webb Rd., Wichita, Kansas was exposed to amputation and laceration hazards in that personal protective equipment was not provided to protect the employee's hands.
