29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system: a) 208 Woodruff Avenue, Avenel, NJ 07001. Employee was exposed to fall hazards when working on the second floor landing in excess of 6 feet above the lower level with no fall protection system in place. b) 208 Woodruff Avenue, Avenel, NJ 07001. Employee was exposed to fall hazards when working on the second floor landing while standing on a four foot A Frame ladder which elevated him above a guardrail system that was incomplete as it was missing a mid-rail and not fabricated with the appropriate size lumber. Incident occurred on or about 05/21/15.
