29 CFR 1926.652(e)(1)(i): Members of support systems were not securely connected together to prevent sliding, falling, kickouts, or other predictable failure: (a) Four Seasons at Monmouth Woods, West Farm Rd., Howell, NJ: On or about 5/21/2015, Whittendal Excavating employees were exposed to potential kickout of trench shield wall due to a missing connection pin in an upper support strut / spreader bar. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
