29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 700 North Front Street, Philadelphia, PA 19123: The employer did not ensure that each employee performing framing work was protected from falling. Employees were exposed to falls of approximately 18ft while walking on the top plate of the building's second floor. Conditions were observed on or about May 18, 2015.  b) 700 North Front Street, Philadelphia, PA 19123: The Employees were exposed to falls of approximately 9ft while assembling an exterior wall on the second floor. Conditions were observed on or about May 18, 2015.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  a) 700 North Front Street, Philadelphia, PA 19125: Employees performing framing were not trained in the recognition, avoidance and mitigation of fall hazards.
