29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could pose a hazard. Permanent aisles and passageways were not appropriately marked: (a) On or about 05/19/2015 - Main Plant, BMM Bathroom Door, the view leading out of the doorway into an area with forklift travel was obstructed by wall lockers and cabinets and permanent aisles and passageways were not marked where forklift trucks and golf carts were being utilized.
