29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): On or about 5/15/2015 at 2149 Fothergill Drive in Evans, Georgia: Employee walking/working on the second level of the single-family home was exposed to fall hazards at heights up to 21 feet 6 inches to the ground level. The employer failed to protect the employee from fall hazards such as but not limited to: 1. Open sides, and ends on the first and second levels of the home. 2. Door opening on the first level of the home. 3. Several window openings were missing a top rail on the first level of the home. 4. Basement floor opening on the first level of the home.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about 5/15/2015 at 2149 Fothergill Drive in Evans, Georgia: The top of the job made portable wooden ladder being used by the employee to access the second level was about 30 inches below the landing.
