An employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Job Site - On or about May 21, 2015 An employee was at the edge of a flat roof while using a rope to hoist material to the roof. Employee was not protected from falling to a lower level by a guardrail system or personal fall arrest system.

29 CFR 1926.1053(b)(4): Ladders were not used for purposes other than the purpose for which they were designed: (a) Job Site - On or about May 21, 2015 an employee used two six-foot fiberglass stepladder to gain access to an upper flat roof level. The employee folded the ladder and leaned it against a wall and then climbed it to the upper flat roof level. The step ladder is not designed to be used in the folded position.
