29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   (a) On or about May 21, 2015 at worksite 501 Conley Ave, Columbia, Missouri, employees were exposed to fall hazards while performing tasks on numerous working platforms of a supported scaffold which were not fully planked or decked.  The highest working platform which was not fully planked was positioned approximately 30 feet above the surface below.    Abatement Note: Abatement certification for this violation is not required.

29 CFR 1926.451(e)(1):    When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   (a) On or about May 21, 2015 at worksite 501 Conley Ave, Columbia, Missouri.-Employees were exposed to a fall hazard  as they climbed the structure of a four section supported tubular framed scaffold approximately 30 feet in height to access the scaffold working surface and was not provided a correct point of access.      Abatement certification for this violation is not required.

29 CFR 1926.451(f)(3):  Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   (a) On or about May 21, 2015 at worksite 510 Conley Ave, Columbia, Missouri.-employees were exposed to fall hazards while working on a four section tubular framed scaffold approximately 30 feet in height, there was not  a competent person onsite to inspect scaffold components for visible defects before the work shift began, or after any occurrence which could affect the scaffolds structural integrity to include repositioning of working platforms, or movement of scaffold sections and components..   Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

1926.451(g)(1)(viii):    Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   (a) On or about May 21, 2015 at worksite 510 Conley Ave, Columbia, Missouri.-employees were performing masonary operations on a scaffold at an elevated height approximately 30 feet without any means of fall protection to include personal fall arrest systems or guardrail systems.    Abatement Note: Abatement certification for this violation is not required.

29 CFR 1926.454(a)(3):   The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training must include the proper use of the scaffold, and the proper handling of materials on the scaffold:   (a)  On or about May 20, 2015 501 Conley, Columbia, Missouri., employees were exposed to serious injuries when they were not trained to recognize the fall hazards associated with the tubular framed scaffold in use on the north side of the work area.   Employees were exposed to approximately 30 ft. fall to the concrete surface below.     Abatement Note: Abatement certification for this violation is not required.
