OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using automotive lifts:    (a)  On or about May 15, 2015, and at times prior the employer did not ensure that employees         were safe from crushing and struck-by hazards in that the following lifts were noted with deficiencies:        (1)  Hydraulic in-ground automotive lift, located in bay 1, dropped while in use and the safety locks were             not operational.                  (2)  The employer did not document that all lift users were trained to use the lifts.        (3) Auto lifts were not periodically inspected by a qualified inspector at least annually to ensure that they were properly maintained and functioning. 29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:    a)  On or about May 15, 2015, and at times prior, employees were exposed to fire hazards while they pumped flammable liquids (brake wash) from a 55 gallon drum, which did not have a grounding strap, into containers which were not bonded to the fill stem. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were not dispensed into containers where the nozzle and container were electrically interconnected:  a)  On or about May 15, 2015, and at times prior, employees were exposed to fire hazards while they pumped flammable liquids (brake wash) from a 55 gallon drum into containers which were not bonded to the fill stem.
