29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system.    (a) Seville Apartments  - On or about May 21, 2015, four employees were working at the edge of a flat roof without the use of any fall protection to protect them from falling and landing on the asphalt parking lot.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    (a) Seville Apartments - On or about May 21, 2015 employees were using an aluminum extension ladder to gain access to the roof without having the ladder side rails extending at least 3 feet above the landing. The ladder was on uneven ground and leaning without being tied off to prevent ladder from falling while entering and exiting the roof.
